I signed a plea bargain in court can a judge suspend it?

ok i signed a plea agreement for 2 fourth degree crimes. I have a criminal record from 12 yrs ago and have not been in trouble since. now i have this charge and since signing the plea my mom has become very ill... she is on life support. Tomorrow is my sentencing and my lawyer told me to come cause she has not heard back from the prosecutor for a post ponement. im scarred im gonna go to jail she said she is confident that it will eitherbe adjourned or the judge will suspend the sentence..... help me
